Arizona State Licensure

All facilities providing custom fabricated prostheses or orthoses must be accredited by an HHS approved accreditation organization​. Also, the custom prosthesis or orthosis must be provided by a Qualified Practitioner, as defined in 42 United States Code Section 1395m(h)(1)(F)​.
