The following fees shall be paid to the (Agency) and are nonrefundable:

The fee for application for a license as an orthotist, prosthetist, prosthetist/orthotist or pedorthist is $___.

The fee for application for a license as an orthotic fitter, mastectomy fitter, therapeutic shoe fitter or assistant is $___.

The fee for application for a license as a technician is $___.

The fee for renewal of an orthotist, prosthetist, prosthetist/orthotist or pedorthist license is $___ per year.

The fee for renewal of an orthotic fitter, mastectomy fitter, therapeutic shoe fitter, assistant license is $___ per year.

The fee for restoration of a license other than from inactive status is $___ plus payment of all lapsed renewal fees. 

The fee for issuance of a duplicate license or for the issuance of a replacement license for a license that has been lost or destroyed is $___. 

The fee for the issuance of a license with a change of name or address other than during the renewal period is $___.  No fee is required for name and address changes on (Agency) records when no duplicate license is replaced.
