What’s the overall cost of accreditation?

The non-refundable base application and survey fees for accreditation are:

New facilities
- $1,525 for primary location
- $900 for affiliate location
- $1,225 for mastectomy-only primary location
- $750 for mastectomy-only affiliate location

Renewing facilities (most accreditations are valid for up to three years)
- $825 for primary location
- $600 for affiliate location

The annual fees are
- $600 for primary location
- $300 for affiliate location
- $300 for mastectomy-only primary location
- $150 for mastectomy-only affiliate location

When is my annual fee due?
Annual fees are due December 1st of each year.  You will receive the first reminder notification from ABC in September.  The annual fee covers your facility from December 1st through November 30th of the following year.

I am no longer applying to be accredited.  Can I get a refund?
All accreditation application fees are non-refundable.

What if I need additional copies of my accreditation documentation?
Any reprint of previously provided information, including letters, surveys, and certificates, requires a $25 reprint fee.  Payment must be received prior to mailing, emailing, or faxing the requested documentation.  You can fax these requests to (703) 836-0838 or you may mail them to 330 John Carlyle St, Ste 210, Alexandria, VA  22314, to the attention of Facility Accreditation.
