Practitioner Certification Application and Instructions
Important: Read the following instructions carefully before completing this application. Incomplete or improperly completed applications will either delay processing and/or render the application ineligible. You are requested to photocopy all forms in advance in the event you need additional copies or make an error. Complete the application and registration form in its entirety.
A) Refer to the Practitioner Certification Book of Rules for information on the criteria for certification and the application process.
B) All requirements, educational and experiential, must be met prior to the application deadline date, March 1, 2007.
C) Applications must be submitted on current forms which are revised for the forthcoming examination session. Dates of the examination session for which the application is applicable are indicated on the forms in the lower right corner.
D) Applications must be legibly written.
E) A completed registration form must accompany your application. New applicants must register for at least one examination at the time of application. Applicants may elect to register for all three examinations within the same exam session. Registrations for the Clinical Patient Management Exam (CPM) are taken on a "first-come, first-serve" basis. The three examinations are independent of each other and scored separately. The exams may be taken in any order.
F) Retain a copy of the completed application and registration form for your records.
G) Enclose the application fee ($250.00) and the appropriate examination fee(s). Make check, certified check or money order payable to The American Board for Certification in O&P. Credit card payments are accepted. Please record method of payment on the payment form.
H) Applications and all requested documentation must be received by the application deadline. Please note: the U.S. Postal Service is the only express service that can deliver to a "Department" address. Also, it is recommended that you do not use "certified" mail.
I) Applications will not be reviewed over the telephone. Applications take approximately six to eight weeks to process from the date of the application deadline. Applicants seeking eligibility under Path 3 (foreign education/ credentials) are advised that such applications must be reviewed by the Applications Review Committee and take a longer time to process. Applicants seeking eligibility under Path 4 (unique qualifications) must use the application specifically designed for that pathway, please contact ABC for the correct form. Applicants will be notified in writing regarding eligibility.
J) The following documentation and fees are required as part of the official application:
The Application
Application Fee ($250.00). The application fee is non-refundable. A $25.00 fee will be assessed for any check or credit card number returned to ABC for any reason. The application fee is required of all applicants, including those extending their ABC credential to the other discipline.
General Information/Education form. Indicate your demographic and educational information on this form.
Experience form. List each separate segment of employment experience, NCOPE Accredited Residency or separate supervisor. This requirement also applies to applicants who may have experienced a change in supervisors, but remained employed by the same facility. The candidate must complete all NCOPE residency program requirements. These requirements include the research paper, a technical attestation, resident evaluations, and all quarterly evaluations. Applicants must complete all of the requirements of the NCOPE residency prior to submission of an application; applicants failing to do so by the application deadline will not be approved for the ABC certification exams.
Official undergraduate transcripts. Transcripts must be submitted from the university/college which conferred your undergraduate degree. Transcripts may be sent directly to ABC from the school or enclosed in official school sealed envelopes mailed with your application. All transcripts must be received by the application deadline. Transcripts are not official if opened by the applicant. Student copies of transcripts are considered unofficial. If you are extending your ABC credential, you do not have to submit a new transcript if your original application was filed in the last 7 years.
Transcripts from orthotics and/or prosthetics certificate program: If applicable, you must submit the school transcript from an NCOPE or CAAHEP accredited orthotic and/or prosthetic certificate program or a photocopy of your certificate.
World Education Services (WES) Report: Applicants who received their undergraduate and/or O&P education outside the United States must submit an evaluation of this education from WES, P.O. Box 745, Old Chelsea Station, New York, NY 10113-0745, (212/966-6311). Please request from the WES an "evaluation of your educational credentials."
