Name: To find an Individual, enter the last name, followed optionally by the first name or initial and choose Individual from the Facility or Individual drop-down list
Example - To find Brad Smith, enter Smith Brad or Smith B, no comma
To find a Facility, enter the first part of the facility name to include the most results and choose Facility from the Facility or Individual drop-down list.
Proximity: Used with zip code to allow you to specify a mileage radius
Accreditation type: Search for a Facility by the area in which they hold an Accreditation.
Discipline: Search for individuals who are ABC certified in a specific field.
